Welcome to our dog walking service! By signing this form, you agree to the following terms:

1. CARE & SAFETY: We will treat your dog with the utmost care and use professional judgment in all situations.

2. EMERGENCY VETS: In the event of an emergency, we will contact you immediately. If we cannot reach you, we will take your dog to the vet listed on this form. All vet costs are your responsibility.

3. CANCELLATION: Please give 24 hours notice for cancellations. Late cancellations may be charged at 50% of the session fee.

4. KEYS & ACCESS: Any keys provided will be kept secure and returned upon request.

5. PHOTOS & UPDATES: We may send photos of your dog during walks. Please let us know if you'd prefer not to receive these.

6. HEALTH: Your dog must be up to date with vaccinations and flea/worm treatments. Please inform us of any health changes.

7. LIABILITY: We are insured for public liability. We cannot be held responsible for pre-existing medical conditions.
